The Patient Centered Medical Home is an exciting new concept revolutionizing primary care in the United States. Our community family health centers are recognized as Medical Homes by the National Committee for Quality Assurance (NCQA).

The medical home is a way to provide you with better, more personal healthcare. It sounds like a place, but it's really a process that helps caregivers collaborate to give you the best care possible.

The team is led by your personal doctor, who gets to know you and makes sure everyone's on the same page. There are also nurses, pharmacists and other professionals, all depending on your personal needs.

When you need a specialist, the team will help find someone right for you. And when you have a problem at night or on the weekend, you can always call for help and information.
