Community-Based Family Health Centers

Each of our five family health centers provides preventive and primary care, as well as specialty care right in your neighborhood. We offer extended hours in a convenient, easily accessible setting (nearly every major train stop is two blocks away or less):

  • Williamsburg Family Heather, located in Williamsburg
  • Manhattan Avenue Family Health Center, located in Greenpoint
  • 61s Street Family Health Center, located in Sunset Park
  • La Providencia Family Health, located in Bushwick
  • Church Avenue Family Health Center, located in Flatbush-Ditmas Park

Hospital-Based Ambulatory Programs

TBHC has several ambulatory programs located at our Downtown Brooklyn hospital campus, including:

  • Ambulatory Care Center, primary and subspecialty care
  • Children’s Health Center, primary and subspecialty pediatric care
  • Women’s Health Center, reproductive, obstetric and gynecologic care
  • Family Medicine Center, comprehensive healthcare for the entire family
  • Dentistry and Oral/Maxillofacial Surgery Center, comprehensive care from preventive dentistry to sophisticated surgery

Level 3 Patient-Centered Medical Homes

Patient Centered Medical Homes (PCMH)Many of the sites within our ambulatory services network, including the Ambulatory Care Center, the Children’s Health Center and all the Family Health Centers, are designated as Level 3 Patient-Centered Medical Homes (PCMH) from the National Committee for Quality Assurance (NCQA). TBHC earned this prestigious recognition by demonstrating delivery of excellent primary care within a team-based environment, as well as a commitment to performance improvement and integrating evidence-based medicine into the electronic health record.

Ambulatory Specialty Care

Program for AIDS Treatment and Health (PATH) Centers

As a New York State Designated AIDS Center, we provide comprehensive ambulatory and primary care services to people with HIV/AIDS. We care for adolescents and adults. TBHC as two PATH Centers, one on the main hospital campus and one located at Church Avenue Family Health Center.

Women, Infants and Children (WIC) Program Centers

TBHC operates seven facilities throughout Brooklyn, including one on the main hospital campus and at 61st Street Family Health Center. Services include supplemental nutrition, education and support to women and their children up to age five.

Rockwell Chronic Renal Dialysis Center

The Rockwell Chronic Renal Dialysis Center located in Fort Greene offers nationally recognized services and a team approach to care for dialysis patients.
