The Brooklyn Hospital Center's newest family health center, located at 2244 Church Avenue, offers a team-based approach to providing comprehensive health care for the entire family. Just steps away from our former location, our Church Avenue center offers an expansion of services, a modern, spacious healthcare environment, and greater access to the care you need.
The Church Avenue Family Center offers comprehensive primary care, with the addition of several new services, including dental, podiatry and gynecology care. Primary care includes preventive care, and screenings and assistance with behavioral health and social services needs. The center cares for the whole person, and provides management of chronic health conditions such as diabetes, high blood pressure, asthma, obesity and HIV. Additionally, our dedicated staff coordinates high quality health care based on proven research and takes responsibility arranging care with other health professionals when necessary, making sure all caregivers are aware of a patient’s medical history, prescriptions, lab test results and radiology images. They offer same-day appointments whenever possible and access to after-hour care. Education, counseling, and self-management options are routinely provided and explanations are made in an easy-to-understand way, with an emphasis on caring for patients with respect, kindness, and sensitivity to religious and cultural beliefs.

The number one priority at the PATH Center is ensuring the highest quality of care for patients in a welcoming and caring environment.
The PATH Center has active Consumer Advisory Boards for both adults and adolescents that help to guide the development and delivery of services. TBHC's PATH Center staff collaborate with more than 60 community based agencies, including substance abuse services, behavioral health providers and home care entities.
The New York State Department of Health awarded PATH the Leadership in Quality Award, and the New York City Department of Health and Mental Hygiene recently recognized PATH for its outstanding quality of care.
Members of the PATH Center team have traveled to India annually since 2005 to provide medical care to patients in resource poor areas.
